Getting started is easy!
Fill out and sign the one page “CO-OP Enrollment”, fax it to us and you’re in!
Start sending/faxing your claims to us right away. You can even send us any claims that didn’t get paid for the past year. We’ll research them and if there is any way to get them paid, we’ll find it.
This is an Open Ended Agreement.
You don’t have to commit to years of forced association. The agreement lasts as long as it works for both of us.
